What Is Pathways to Community Living Grant in Oklahoma?

The Pathways to Community Living grant is a federal grant program in Oklahoma that’s also known as the Money Follows the Person Tribal Initiative.

This grant:

  • Helps elders and individuals in tribal communities move out of nursing homes and back into their homes

  • Focused on long-term, culturally appropriate support that promotes independence and dignity

  • Works in partnership with Oklahoma Health Care Authority (OHCA) and tribal governments

 

 

 

 

Who is eligible for the Pathways to Community Living grant?

  • Tribal members currently living in nursing homes or rehab centers

  • Individuals who are interested and medically eligible to transition back into their community

  • Must be enrolled in SoonerCare (Oklahoma Medicaid)

  • Tribal Nations and organizations serving eligible tribal members

  • Tribal Nations or affiliated health programs in Oklahoma

  • Organizations looking to expand or support home- and community-based services

  • Groups ready to partner with OHCA to offer sustainable long-term care solutions

 

What Are the Conditions of the Pathways to Community Living Grant?

  • No additional contracts are needed—tribes work directly with OHCA

  • Services are flexible and tailored based on tribal input

 

This grant can be used to:

  • Lease vehicles for community transportation (not just medical trips)

  • Modify homes for safety and accessibility

  • Hire staff or add new support services

  • Reimburse services your tribe already offers (like meals, counseling, or home visits)

  • Provide medical equipment to help people live at home safely

 

Final approved services may include:

  • Case management

  • Skilled nursing

  • Mental health counseling

  • Adult day services

  • Personal care and self-directed services

  • Housing-related transition support
