Who can apply for the Rural Health Care Services Outreach Program Grant?
Eligible applicants include domestic public, private, non-profit, and for-profit entities. This includes, but is not limited to:
- Non-profits with 501(c)(3) status or other IRS-recognized non-profits.
- Public and private institutions of higher education.
- State and county health departments.
- Hospitals, including rural emergency hospitals.
- Native American tribal governments and organizations.
- Community-based organizations.
- Federally Qualified Health Centers (FQHCs).
- Rural health clinics.
Tribal exception
- Tribes or tribal organizations that do not meet the Employer Identification Number (EIN) or Unique Entity Identifier (UEI) requirements can request an exception.
- This exception applies to those with only one EIN or UEI or those unable to meet the three-entity consortium requirement.
- To apply, tribes must submit a Tribal EIN Exception Request as part of their application.
